The Instructions for completing the “working papers” are at the end of this document . Please ensure that Sections A, B and C are complete prior to bringing your working papers in for certification.
Dismissal Document
As part of the school’s safety and security plan parents/guardians are required to provide school personnel with information pertaining to pick up instructions during dismissal.
Student Emergency Contact
As part of the school’s safety and security plan parents/guardians are required to provide school personnel with an emergency contact. If there is a medical emergency, the student will be taken to the nearest hospital.
Late Pick-Up Notice
School is dismissed at 3:45 pm Monday – Thursday and 1:30 pm on Fridays. There is no aftercare program and there is no supervision available for late pick up students. All students who are not picked up by 4:00 pm Monday – Thursday and 1:45 pm on Fridays will be charged $1.00 per minute until the child is picked up.
Release of Records
This form is to request for a release of student records to CAPS Central, including gifted, intervention, educational, medical, social or special education information.
Student Information Update
It is important that we have accurate records if anything has changed. This form entails very basic information on your student’s and your emergency contacts.
Wellness Assessment
The Wellness Assessment provides information on wellness policy goals and practices within the school district. It is used to track progress and gather ideas on ways to create a healthier school environment.

The Asthma Treatment Plan is designed to help everyone understand the steps necessary for the individual student to achieve the goal of controlled asthma.
Universal Health Form
Student medical exam neededThis form outlines the required information needed including immunization, medical conditions, medications, allergies, special diets, mental or behavioral issues, as well as other health information.
Non-Food Allergy Action Plan
This form is for non-food allergy emergencies and should be completed by a healthcare provider.
Food Allergy & Anaphylaxis Emergency Plan
This action plan form outlines what to do in the event of severe or mild symptoms as well as instructions for various injectors including EpiPen®, AUVI-Q™, and Adrenaclick®.
